Mechanism of Laser Tattoo removal
Tattoos consist of thousands of ink particles in the skin. Laser energy with peak power is delivered on the tattoo which is absorbed by the tattoo ink. This generates a shock wave which is propagated throughout dermis causing the tattoo ink to fragment which is then cleared by the macrophages.
Pre-laser preparations:
A test patch is usually done to determine the best parameters for an individual.
Protective eyewear is used by patient and doctor both.
Topical eutectic mixture of local anesthetic (EMLA) is applied under occlusion 45 minutes prior to the procedure.
Pre laser pictures are taken for comparison.
Laser wavelength, fluence, spot size are set as per the tattoo type (amateur or professional), color (black, blue, red etc.), age (old or new) etc.
The Procedure:
Laser treatment is performed on the tattoo during which a popping sound is heard due to the photoacoustic effect of the laser.
Endpoint of laser is the immediate whitening of the lesion. Higher fluences may produce bleeding and blistering.
R20 or R0 Method may be performed if needed.
Number of sessions:
Tattoos may need 2-10 sessions for successful lightening. The number of sessions needed depends on various factors like tattoo type, age, intensity, layering, etc. Sessions should be done at least 6-8 weeks apart.
Post laser care:
A good sunscreen is to be applied regularly throughout the treatment period
Post laser, a little burning is common-Apply icepacks till the sensations subside and a layer of antibiotics cream is applied.
Patients are advised to apply an antibiotic cream and moisturizer for about a week after the procedure.
Crusting will appear on the lesion post-laser in a day or two which may take 5-10 days to heal. Do not try to pick the crusts and remove them.
Anti-inflammatory analgesics may be used if large areas are treated.
